The BrainHealth GUIDE Program was built for moments like these.
-
“I haven’t had a day to myself in months.”
-
“Mom won’t take her pills, and I don’t know who to call.”
-
“It’s 2 a.m. and Dad is awake and upset. I don’t know what to do.”
-
“What do I do when he refuses a bath, or won’t settle at night?”
“When my husband was first diagnosed with Alzheimer’s, I felt lost… They treated my husband with dignity and respect, while also giving me the knowledge I needed… Thanks to BrainHealth, I no longer feel like I am facing this journey alone.”
Up to65 hoursof respite care a year
Time to take care of you, so you can keep taking care of them.
- Your own doctor’s appointment
- A haircut
- Lunch with a friend
- A nap
About 4 hours a week, for four months, or however works for your family.
A trained helper comes to your home. We arrange it and pay the agency directly; you file nothing. For families caring for someone with moderate or advanced dementia.
For your loved one
Their own Care Navigator, the same person every time you call. In a typical week, they might schedule the neurology follow-up, call the pharmacy about a new prescription, or find an adult day program nearby. One call instead of five.
A written care plan with who to call and when, what changes to watch for, the medication schedule and upcoming appointments, built with your family and shared with their doctors.
Medication check-ups so prescriptions aren’t making confusion worse.
Care that works with their current doctors, no switching needed.
For you, the care partner
A 24/7 phone line. At 2 a.m., when Dad won’t settle, you reach someone who already knows his care plan, not a call center.
Caregiver training on the hard stuff: bathing, wandering, evening restlessness.
Help finding local resources: adult day programs, meals, rides, support groups.
A seat at every visit. Everything is by phone or video, and you’re welcome on every call.

A note from our Chief Medical Officer
Dementia care shouldn’t fall on one person.
Families caring for someone with dementia carry an enormous load, usually without a break and often without anyone to call. GUIDE exists to change that, and Medicare pays for it. My team’s job is to make it simple: one short call, and we take it from there.
You’ve been carrying this long enough.
Share a few details and we’ll help you find the right program for your family, GUIDE or the BrainHealth Clinic. You can fill this out for yourself or on behalf of the person you care for. Prefer the phone? Call (916) 314-8600.

Questions families ask
GUIDE (Guiding an Improved Dementia Experience) is a Medicare program run by the Centers for Medicare & Medicaid Services (CMS) for people with dementia who have Traditional Medicare. Enrollment is voluntary, you may leave at any time, and it does not change your Medicare coverage or your doctors. Respite is available to patients with moderate or advanced dementia who have an unpaid caregiver, up to Medicare’s annual limit. The statements contained in this document are solely those of the authors and do not necessarily reflect the views or policies of CMS. Eligibility requirements apply. Not all individuals qualify. Note: this is not medical advice.
